  2. Completed Family Composition - Form 54
  3. Completed Financial Analysis - Summary Sheet
  4. A Letter of Offer of M.S. Program from .....University
  5. A Letter of Offer of English Program from .....Institute
  6. Official score report of IELTS
  7. Resume
  8. Purpose statement
  9. Notarial degree certificate, and diploma
  10. Official academic transcripts from my undergraduate university
  11. Copies of the academic awards and honors of my undergraduate studies
  12. A letter of recommendation from my professor
  13. A copy of each page of my family registration book
  14. A copy of each ID card of my family members
  15. Notarial certificate of birth
  16. Notarial certificate of family relationship
  17. Financial support statement
  18. An original letter of approval and recommendation from my company
  19. An original letter from my employer, showing my position, salary and other forms of income received and the date of commencement
  20. Original salary slips from last 2 months
  21. Introduction brochure about my company
  22. Copies of the letters of my job promotion
  23. An original letter from my father's company detailing his position, salary and other forms of income received and the date of commencement
  24. A copy of my father’s recent salary slips
  25. Official certification of deposit
  26. A photocopy of bank slip
  27. A photocopy of stock transaction list
  28. Four (04) completed self adhesive name and address labels in Chinese characters
  29. Four (04) passport sized photographs
  30. Notarial certificate of no-criminal record
  31. My declaration
